Please Help Us Design Our Surveys

We are currently designing our survey. We want to ask relevant questions, meaningful, and forward thinking questions. We need your help. We are just 2 guys, and although we have our ideas of where we are, where we are coming from and where we could go, we recognize that we are just 2 perspectives, and so we need yours.
